Once you have registered, either online or via mail, you will receive
a confirmation email from Jesse Klein.

If you have registered, but have not received the email,
check with Jesse Klein to confirm your registration.

A medical release form can be found HERE

The Medical Release Form must be completed and signed by a physician and returned to the Cyclone Volleyball Office. In lieu of a physician’s signature on the release form, you may turn in a physical that is less than one year old.

***It is preferred you return the form prior to camp. If you are unable to send in the form before camp, please bring the completed form to the first day of camp.
